In compliance with Texas House Bill 4224 and Section 181.105 of the Texas Health and Safety Code, Texas-licensed behavioral health providers must provide consumers with detailed instructions on their rights and complaint options.
Records Requests
You have the right to request copies of your protected health information (PHI).
To obtain a copy of your records, you must submit a written, signed request via our secure portal or email us at admin@abidefortworth.com.
We will fulfill your request or provide a formal response within 15 business days of receiving your written submission, in accordance with Texas law.
